Medical Benefits Overview

Ascent Aviation Services offers three medical plan options through United Healthcare (UHC) Choice Plus network which provides an option for you and your family to choose from:

  • All three plans are open access plans and do not require a referral to see a specialist and have in and out of network coverage
  • All three plans cover the same diagnosis, procedures and treatments but at different amounts
  • The three medical plan options are – two PPO ($1,000 and $1,500 Deductible Plan) and High Deductible Health Plan (HDHP) which includes a Health Savings Account (HSA) benefit. The HDHP is a great plan for someone who has minimal medical claims and wants to save money on premiums and can rollover their health savings account.
  • The difference between the plans is how much you pay in premiums vs. how much you pay at the time of service. This allows each employee to choose a plan that is most economical for their individual and family needs.
  • Get the UHC mobile app for quick and easy access to your plan information including ID cards, in-network providers and how much of your deductibles and out of pocket maximum have been satisfied.

Dental Overview

Ascent Aviation Services offers two different PPO dental plans through United Healthcare (UHC) which provides options for you and your family to choose from:

  • United Healthcare Dental (Low Plan and High Plan)
  • Employees are free to choose to see any dentist they wish, even out-of-network (with reduced benefits).
  • In-network providers will file their claims for you.

Vision Overview

Ascent Aviation Services offers a vision plan that is administered by United Healthcare (UHC).

  • The vision plan is a voluntary benefit paid by the employee.
  • The plan offers in-network exams and materials on a co-pay schedule along with out-of-network benefits at a set reimbursement schedule.
  • In-network providers will file their claims for you.

  • Also, the program has additional services at a discounted rate.

Life/AD&D Overview

Ascent Aviation Services offers Basic Life, Supplemental Life and AD&D Insurance options for you, your Spouse and/or Child(ren) through United Healthcare (UHC).

  • Basic Life & Accidental Death and Dismemberment (AD&D) Insurance is provided to you at no cost. The benefit amount offers to you is depending on your class.
  • Supplemental Life and Accidental Death & Dismemberment Insurance is coverage that you pay for and pays your beneficiary a benefit if you die while you are covered or the AD&D will provide benefits due to certain injuries or death if from an accident Below are the Supplemental Life and AD&D Insurance options for you, your Spouse and/or Child(ren).
    • Employee Supplemental Life AD&D may be purchased by you in increments of $10,000. The maximum amount you can purchase cannot be more than the lesser of 5 times your annual earnings or $500,000. Evidence of Insurability (EOI) is required for all elections above $200,000. Please note that if you don’t elect this coverage when you are first eligible for it, any amount you elect later will require EOI.
    • Spouse Supplemental Life & AD&D may be purchased by you in increments of $5,000, to a maximum of $250,000. Coverage cannot exceed 50% of the amount of Employee Supplemental Life and AD&D Insurance coverage. Evidence of Insurability (EOI) is required for all elections above $50,000. Please note that if you don’t elect this spouse coverage when you are first eligible for it, any amount you elect later will require EOI.
    • Child Supplemental Life may be purchased by you in increments of $2,000 to a maximum of $10,000 for each Child - no medical information is required. However, coverage cannot exceed 50% of the Employee’s Supplemental Life and AD&D amount. Please note that this benefit is limited to $10,000 for a child age 14 days to 26 months. No benefit is paid for a child under 14 days old. The cost is the same whether you cover one or several eligible children.

Short Term Disability

Ascent Aviation Services offers Short-Term Disability Insurance through United Healthcare (UHC) and it is paid for by the company.

  • The coverage provides income replacement if you are sick or injured (non-occupational) and cannot work.
  • The Weekly Income Benefit is 60% up to a certain weekly maximum.
  • The Elimination Period and Maximum Duration of Benefits varies by class.

Long Term Disability

Ascent Aviation Services offers Long Term Disability Insurance through United Healthcare (UHC) and it is a voluntary benefit that is paid for by the employee.

  • Monthly Income Benefit:
    • 60% of your monthly earnings up to $5,000.
    • Benefits will never be less than the greater of 10% of your monthly benefit or $100.

  • Maximum Duration of Benefits:
    • Benefits are payable following the elimination period and for the remaining period of total disability.

Employee Assistance Program

Ascent Aviation Services offers all benefit eligible employees with Work-Life Benefits through an Employee Assistance Plan (EAP), at no cost to you. Services for this plan are offered through Cascade Centers. The program allows employees and their household members to receive assistance with a wide range of life's challenges such as short-term counseling, information on community resources and programs, educational referrals and materials. The plan offers:

  • Confidential services such as counseling, conflict resolution, workplace effectiveness training, behavioral health, and employee assistance program consultation for the Ascent Aviation Services employees and dependents.
    • Each employee or their dependent is allowed 6 counseling visits per nature of the visit.
    • Unlimited telephonic consultation with a qualified EAP representative.
